High School Transition Office

Transition planning includes a coordinated set of activities designed within an outcome-oriented process, that promotes movement from school to post-school activities. These outcomes include:

• Post-secondary education
• Job training
• Continuing and Adult education
• Integrated employment
• Adult services
• Independent living
• Community participation

Students receive a variety of services to help them attain their identified goal(s). These services may include, but are not limited to: career education and assistance, social skill and self-advocacy instruction, technology education, in-school and community internships, on-the-job training, independent living skill instruction, and linkage to community agencies.
