All new students, Pre K or K, 1, 3, 5, 7, 9 & 11. Any physical done within a calendar year of school start (which is July 1), is acceptable if done within NYS or by an Army provider. We do accept any form these physicals come in. Often the Primary Care Provider (PCP) has electronic records and it is easier for them to copy these examinations, which is fine with LCSD. We do have a physical form if you prefer to have it filled out by your PCP. - Physical form 2023
