Physicals

  • All new students, Pre K or K, 1, 3, 5, 7, 9 & 11.  Any physical done within a calendar year of school start (which is July 1), is acceptable if done within NYS or by an Army provider.  We do accept any form these physicals come in. Often the Primary Care Provider (PCP) has electronic records and it is easier for them to copy these examinations, which is fine with LCSD.  We do have a physical form if you prefer to have it filled out by your PCP. - Physical form 2023

    **Health certificates are required to be submitted to the school within 30 days of the start of the school year [Education Law §903, Commissioner’s regulation §136.3 (c)(1]]. If the parent/guardian has not furnished a health certificate to the school within the first 30 days of attendance, school officials must provide a written notification to the parent/guardian that if the parent/guardian does not provide the school with a health certificate within 30 days from the date of the notice, that the school will conduct a health examination of their child at school by the district medical director pursuant to Education Law §903(3)(a) and Commissioner’s regulation §136.3 (c)(1)(iii).

    **Education Law §903 is an explicit statutory exception to the parental consent requirement and consent is not required when the school is conducting an examination by health appraisal as required in Education Law §903 and Commissioner’s regulation §136.3(c) and (d) (and see 20 USC 1232h[c][4][B][ii] (federal statute relating to student surveys, analysis or evaluations contains exception that certain provisions do not apply to “any health examination or screening that is permitted or required by an applicable State law, including health examinations or screenings that are permitted without parental notification”).

    Sports Physicals- All students in 7th through 12th grade who are interested in playing competitive sports at LCSD will need a physical provided by their Healthcare Provider.  If you have done a physical at your healthcare providers, please bring in a copy.  Please inform the athletic department or the school nurse that your student is interested in playing and they will advise you when the sports physical can be done at school if needed. - Physical form 2023

    **Athletic Placement Process - Athletic Placement Process (APP) is a program for evaluating students who want to participate in sports at higher or lower levels, as approved by the Board of Regents as a part of the school eligibility rules in 1980, and is aligned with Learning Standards 1 and 2.
